For Local Students:
- Photocopy of both parents’ ID Card (KTP)
- Photocopy of guardian’s ID Card (if child living with guardian)
- Photocopy of Family Card (KSK)
- Photocopy of child’s Birth Certificate
- Photocopy of child’s Report Book
- Photocopy of Marriage Certificate
- 10 pcs. of color photographs (3 x 4) cm
For Foreign Students:
- Photocopy of parents’ and child’s KITAS
- Photocopy of parents’ and child’s Passport
- 10 pcs. of color photographs (3 x 4 cm)
